St. Charles, Mosaic partner to boost primary care

Mosaic Medical and St. Charles Health System said Monday they are working together in a new partenrship “to enhance primary care services in Central Oregon as part of their commitment to improving the overall health of the community.”

“This innovative partnership is in stride with local, state and national efforts to achieve the Triple Aim goals of better health, better care and better value,” the announcement stated.

In a joint news release, the two organizations said they have signed a letter of intent to begin jointly exploring a broad spectrum of initiatives, including:

Sharing knowledge as both organizations implement Patient-Centered Primary Care Home (PCPCH) practices to help better coordinate all aspects of a patient’s care
Joint projects to reduce the number of uninsured in the region by approximately 11,000 in the next few years Exploring the opportunity to have Mosaic’s non-profit Federally Qualified Health Centers on the St. Charles Redmond and Mountain View Hospital campuses
Creating a robust joint care program to help improve the coordination of primary care for patients who seek non-urgent treatment in the St. Charles Emergency Department and Immediate Care

In addition to a commitment to enhancing the overall health of the community, St. Charles and Mosaic are jointly implementing these initiatives in anticipation of a steep increase in the number of people eligible for Medicare and Medicaid.

Under the Affordable Care Act, Medicare enrollment in Central Oregon is expected to swell by 16,000 people — and Medicaid, by more than 11,000 — by 2014.

“Mosaic Medical is committed to finding new ways of bringing quality primary care to everyone who needs it in Central Oregon, especially our most vulnerable populations,” said Megan Haase, Mosaic’s chief executive officer. “We are looking forward to this increased partnership with St. Charles to benefit our patients and our region.”

Making sure patients receive the right care at the right time will help keep them healthy, and contain or drive down their cost of care, said Dr. Jeff Absalon, St. Charles’ chief physician executive.

“St. Charles and Mosaic Medical are well-aligned,” he said. “We’re both committed to caring for our community and focusing on personal health. We are excited to work together to share our collective expertise as we craft new models for optimal health care delivery.”
